How pornography can affect erectile function in men

BY DR. PAULO EGYDIO · CRM-SP 67.482 · RQE 19514

Problematic pornography use may be associated with difficulties in sexual health, including performance anxiety, comparisons and unrealistic expectations, which can also affect relationships.

Watching pornography does not mean a man will develop erectile dysfunction. However, problematic pornography use may be associated with sexual difficulties, partly for psychological reasons, such as unrealistic expectations about appearance and sexual performance.

Research investigates whether does pornography cause erectile dysfunction, but it does not allow concluding that consuming pornography, by itself, causes erection problems.

In addition, erectile dysfunction may be related to different issues, such as hormonal dysfunctions, cardiovascular diseases, side effects of some medications and psychological problems, such as depression.

Therefore, it is important to carry out a personalized analysis to identify the causes of erectile dysfunction.

Does pornography cause erectile dysfunction?

A “longitudinal study on pornography and erectile dysfunction published in 2019 was the first to analyze the relationship between problematic pornography use by the user himself and erectile dysfunction. The research did not include medical factors associated with erection difficulties.

The authors followed the study participants for one year and did not find evidence that excessive pornography consumption causes erectile dysfunction. The authors conclude that there may be an association between these factors and that psychological problems may also influence.

What is the relationship between pornography and erection — does pornography cause erectile dysfunction?

The relationship between pornography and erection may involve psychological and emotional factors, especially when there is problematic pornography use, performance anxiety or unrealistic expectations about appearance and sexual activity.

Problematic pornography use

A “systematic review published in 2026 analyzed 11 studies on pornography and male sexual dysfunction.

The authors emphasize that problematic pornography use shows a greater association with erectile dysfunction and other sexual dysfunctions

The concept of ‘problematic’ is not related only to the number of hours or to the frequency of consumption. Use may be considered problematic when it involves aspects such as:

  • Loss of control: difficulty deciding when to start consumption or to stop;
  • Difficulty reducing the behavior: repeated attempts to decrease or stop use;
  • Interference in personal life: pornography begins to take up time and attention that should be devoted to work, study, leisure and relationships;
  • Impairment in relationships: the behavior interferes with the romantic relationship, reducing availability for spending time together and intimacy;
  • Significant distress: causes psychological distress or disruption to routine;
  • Use as a recurring strategy to cope with emotions: pornography becomes a tool to cope with stress, anxiety, loneliness, sadness, or other negative emotions.

Performance anxiety

The performance anxiety is not limited to the sexual context. It can occur in the face of fear of failure or of not meeting expectations in situations such as studying, work, or public speaking.

In sexuality, this type of anxiety can arise when a person sets unrealistic expectations about their own appearance or performance, and begins to compare themselves with friends or performers in adult content, starting to pressure themselves excessively.

This fear of failing can increase worry during the encounter and make it difficult to focus on sensations and pleasure, interfering with the sexual response, including the ability to achieve or maintain an erection — raising the question, does pornography cause erectile dysfunction?

Expectations about body and sex

Pornography consumption can trigger unrealistic expectations about appearance and performance. Comparing oneself with the performers’ bodies and performances shown in this type of content can lead some people to see their bodies as inadequate and to view their sexual performance as unsatisfactory.

These negative perceptions contribute to anxiety about sex, impairing sexual performance.

Relationship with sexual desire and satisfaction (does pornography cause erectile dysfunction)

A systematic review and meta-analysis published in 2024/2025 evaluated the possible relationship between pornography consumption and sexual satisfaction, finding heterogeneous results among the studies.

The study started from the premise that dissatisfaction with sexual performance can lead to excessive use of pornography and set out to investigate whether consumption of this type of content can bring any level of satisfaction.

The authors analyzed 41 studies, which gathered 70,751 participants, men and women. Although a relationship was observed in the analyzed data, the result was not statistically significant, and therefore does not allow concluding that pornography consumption increases or decreases sexual satisfaction.

How can you tell if erectile difficulty may be related to pornography? (does pornography cause erectile dysfunction)

Understanding the problem, the next step is to seek professional help. Remember: any medical and therapeutic approach must be personalized and recommended according to each patient’s specific situation.

If there is concern about whether does pornography cause erectile dysfunction, this should be evaluated as part of the clinical assessment.

Below, we list some approaches that may be recommended by the physician or therapist according to the patient’s needs:

  • Reduce or stop pornography consumption: in some cases, this change may be part of the approach to sexual difficulties, especially when problematic use exists. Progress varies and, if symptoms persist, other strategies may be considered after professional evaluation.
  • Cognitive-Behavioral Therapy (CBT): CBT can help to understand and modify thoughts related to pornography.
  • Sex therapy: a sex therapist can provide specific strategies to deal with dysfunctions.
  • Couples therapy: often, the problem can also affect the relationship. In this case, couples therapy can help strengthen communication.
  • Anxiety-reduction techniques: when there is performance anxiety, techniques such as mindfulness, and other anxiety-reduction practices may be helpful.
  • Medication: if necessary, the urologist may also prescribe medications in conjunction with other treatment strategies.
Situation What it may indicate
Erection occurs normally in other situations May suggest that the difficulty is situational, especially when it occurs only in certain contexts
Erection occurs alone, but not with a partner May be related to performance anxiety, emotional factors, or the context of the sexual relationship
Difficulty occurs in all situations It is important to investigate possible physical, medication-related, psychological causes, or a combination of these factors
Problem began alongside changes in sexual behavior It is worth assessing whether there was a temporal relationship with changes in the frequency, context, or manner of pornography consumption
There is intense anxiety before sexual intercourse It may indicate performance anxiety, especially if the fear of failing increases worry about the erection
There is loss of control over consumption It may indicate problematic pornography use, especially when there are unsuccessful attempts to reduce consumption

It is important to emphasize that getting an erection during masturbation does not mean that erectile difficulty during sexual intercourse is caused exclusively by psychological factors. Although emotional and behavioral aspects can influence the male sexual health, a medical evaluation is necessary to identify the possible factors involved.

Does stopping watching pornography improve erections? does pornography cause erectile dysfunction

Reducing or stopping pornography may contribute to improved erectile function in some men. However, there is no universal timeframe to determine how long it might take to feel this effect. The question does pornography cause erectile dysfunction remains under study.

In cases of problematic use, the person may have difficulty reducing or stopping consumption without support. A psychological approach may be considered to address this behavior pattern when clinically indicated.

It is also important to investigate possible medical factors associated with erectile difficulties and other changes in sexual function.

Can pornography cause permanent impotence? does pornography cause erectile dysfunction

There is no evidence that pornography consumption causes permanent erectile dysfunction.

When erectile difficulty is persistent, the EAU guidelines recommend assessment of risk factors and addressing the causes and modifiable factors of erectile dysfunction.

These causes include cardiovascular or hormonal changes, as well as reviewing the use of medications that may affect erection.

When does erectile difficulty need medical evaluation?

Erectile difficulty requires medical evaluation when there is:

  • Persistent difficulty;
  • Progressive loss of rigidity;
  • Problem in different situations;
  • Significant reduction in libido;
  • Presence of diabetes, hypertension, or cardiovascular disease;
  • Use of medications that can affect sexual function;
  • Hormonal changes;
  • Pain or anatomical alteration of the penis;
  • Significant psychological distress.

How to treat erectile dysfunction? — does pornography cause erectile dysfunction

The EAU guidelines emphasize the importance of individualized treatment of erectile dysfunction, taking into account each patient’s clinical and personal particularities.

Lifestyle changes, treatment of associated conditions, psychotherapy, oral PDE5 inhibitor medications or intracavernous injections are part of first-line treatment for erectile dysfunction.

Pornography and compulsive sexual behavior — are they the same? (does pornography cause erectile dysfunction)

No. Consuming pornography does not mean having compulsive sexual behavior. (does pornography cause erectile dysfunction?) Consumption can be part of sexual life without representing loss of control or harm.

Compulsive behavior involves a persistent difficulty in controlling sexual impulses and behaviors, even when these cause negative consequences in professional, personal, academic or family life.

The compulsive sexual behavior disorder is a specific condition recognized in ICD-11 (International Classification of Diseases), and the sexual medicine literature warns against the risk of considering sexual behaviors as diseases simply because of frequency or morality.
