Does the Post-Operative Period of the Penile Prosthesis Hurt?

The post-operative experience varies from patient to patient. Some feel more pain, others less. Because we perform an anesthetic block at the base of the penis, this helps control pain for a long time after surgery. It is also possible to use analgesics prescribed by the surgeon if the pain is more intense. In most cases, the pain does not usually reach very high levels, but each patient may feel it differently.

Penile Temperature: Does the Penile Prosthesis Make the Penis Cold?

We always need to evaluate penile sensitivity via erotic arousal, and the good news is that, in the vast majority of cases, it remains exactly as it was before the implant.

Will My Partner Notice?

This possibility is quite remote, even among patients who already had a stable partner before the penile prosthesis. Behaving naturally, as you always have—whether you have an implant or not—is the best way to achieve proper arousal and enjoy your sexual life again.

How to Conceal the Penile Prosthesis?

The prosthesis is not visible, but you must know how to position it correctly. The inflatable or hydraulic model can be deflated, leaving the penis flaccid, which makes it easier to position under clothing. Meanwhile, the malleable prosthesis requires you to position the penis in a way that is most comfortable for you and wear a tighter undergarment or briefs, holding the penis close to the body and making it more discreet.

Is the Penis Always Rigid?

As mentioned before, with an inflatable prosthesis, you can depressurize the cylinder and leave the penis flaccid, but a malleable prosthesis does not work that way. With this model, the penis appears elongated but is easy to adjust and position. If the cavernous sponge tissue is preserved during surgery, the man may still have some residual filling when aroused, even with a malleable prosthesis. This phenomenon is similar to an erection, only this time with the penile prosthesis.

Does the Penile Prosthesis Increase Penis Size?

Both the inflatable and malleable implants are sized to the erect length of the penis so that it has sufficient support during intercourse. The penile prosthesis does not increase penis size, but if the patient is indicated for prosthesis surgery, the penile tissue can be reconstructed via multiple relaxing incisions to expand it, allowing for a larger and wider implant. When the prosthesis is placed in a penis with Peyronie’s disease or congenital curvature—meaning it has a reduced size—in patients with curvature or prior reduction, it may be possible to recover length, respecting anatomical and safety limits.

What About Penile Girth? Does It Get Thicker or Thinner?

We also must consider the penis thickness. If there is penile thinning, we need to identify its type: total, partial, indentation, or hourglass shape. The surgeon must perform tissue expansion in the affected areas by making multiple geometrically calculated relaxing incisions before inserting the prosthesis. It is also very important to select a prosthesis whose length and girth are appropriate for the reconstructed penis. This ratio ensures better vertical rigidity without risking bending or displacement during intercourse. Inflatable Prosthesis: Does Pumping the Cylinder Hurt?

Rest assured, pumping the cylinder of your inflatable prosthesis won’t be painful! The pump is placed in the scrotum and can be operated without applying a lot of force. After the post-operative period, you won’t feel pain. However, before opting for this prosthesis, it is necessary to evaluate factors such as the size of the patient’s scrotum, as well as hand strength and dexterity.

How About Ejaculation and Orgasm After the Penile Prosthesis?

Ejaculation is a mechanical response. It involves muscle contractions in the penile region to expel semen during orgasm, which is the brain-driven sensation responsible for that peak pleasure during intercourse. The prosthesis provides good penetrative capability, but you must become aroused—through kissing and caresses—to achieve orgasm! This equipment helps diagnose various medical conditions, especially in soft tissues like the brain, muscles, internal organs, and joints. During the exam, the patient is placed in the machine, which is essentially a magnet that can attract metallic devices such as implants or pacemakers. For patients with a penile prosthesis, the exam is usually safe for two reasons:

  • Compatibility with MRI: the manufacturer’s documentation states whether the prosthesis is compatible or incompatible with this type of scan.
  • Prosthesis material: modern inflatable and malleable penile prostheses typically contain a limited amount of metal components and are coated in silicone.

When undergoing the exam, it is mandatory to inform the radiologist about the presence of the prosthesis.
