PENILE PROSTHESIS FOR ERECTILE DYSFUNCTION: COMPLETE GUIDE

Penile implantation is indicated in severe cases of erectile dysfunction and also when it is associated with changes in penile anatomy.

The penile prosthesis is a therapeutic alternative indicated for men who do not achieve a satisfactory response to clinical treatments, such as oral or injectable medications.

The procedure may also be considered in cases of marked penile curvature, narrowing, or reduction in penile size, when the deformity causes sufficient structural instability to hinder or prevent penetration during sexual intercourse.

The main objective of the implant is to restore the penile rigidity required for penetrative function, while respecting anatomy, clinical history, and the individual characteristics of each patient, with a focus on functional recovery and quality of life.

What is erectile dysfunction

Erectile dysfunction is characterized by difficulty in achieving or maintaining an erection sufficient for sexual intercourse.

Isolated episodes are common in situations of fatigue, anxiety, or stress, without necessarily representing a health problem.

However, persistent loss of rigidity and reduction, whether associated or not with the absence of morning erections for a period of three months or more, may indicate erectile dysfunction and warrant medical evaluation.

What is a penile prosthesis and who is it indicated for

Penile implantation is a treatment for severe erectile dysfunction and anatomical changes that result in loss of penile functionality.

The indication for a penile prosthesis in erectile dysfunction occurs when the man does not respond or ceases to respond to oral or injectable medications.

Men with severe diabetes, prostate removal, and Peyronie’s disease — a condition characterized by penile curvature — may also require penile implantation.

Types of penile prosthesis

Inflatable or hydraulic prosthesis (2- or 3-piece)

The inflatable prosthesis may have 2 or 3 components; the difference is that the 3-piece model includes a pressurizing pump positioned in the scrotum.

Malleable or semirigid prosthesis

The malleable prosthesis consists of two metal rods coated with silicone.

Articulating prosthesis

The joints of the articulating prosthesis facilitate handling but provide limited rigidity.

Does a penile prosthesis work? What changes after implantation

A penile prosthesis can be an effective alternative when properly indicated. In general, sensitivity and the ability to achieve orgasm tend to be preserved, although this may vary according to individual factors and the type of procedure performed.

In clinical practice and in studies, many patients report improved penile rigidity after implantation, which may positively reflect on confidence and sexual life. Results, however, depend on clinical conditions, surgical history, and the adaptation process to the device.

Penile prosthesis vs. other treatments

Oral medications assist erection but depend on the presence of sexual stimulation to work.

Penile injections, applied directly into the penis, also act by facilitating erection through pharmacological mechanisms.

Penile implantation, on the other hand, is considered when drug-based treatments do not provide an adequate response or in cases of erectile dysfunction associated with anatomical changes that compromise penile function.

What Is Penile Prosthesis Surgery Like?

Penile prosthesis surgery aims to recover, as much as possible, penile dimensions compromised by curvature, reduction in size, or narrowing.

Pre- and postoperative care: what the patient needs to know

Before surgery, the urologist performs a thorough evaluation to define the best surgical strategy. This assessment may include:

  1. palpation and measurement of the penis;

  2. pharmacologically induced erection;

  3. penile Doppler ultrasound to assess blood flow in the region.

The recovery period must be followed strictly and includes:

  • attendance at follow-up appointments;

  • proper wound care;

  • return to work within 7 to 10 days, provided there is no physical strain;

  • return to physical activities, generally after 30 days.

Return to sexual activity varies according to each patient, occurring in most cases between 45 and 60 days after surgery.
